Self Directed Labs

BE IN CONTROL OF YOUR HEALTH

Self Directed Labs allows individuals to request laboratory tests and receive results without a provider's order or insurance involvement.

What you need to know:

  • A provider's order is not required for these tests.

  • Anyone under the age of 18 must be accompanied by a parent or guardian.

  • You must pay for the tests at the time of service by cash or check. We will not bill your insurance. Credit Card not accepted.

  • The results will be sent to you in the mail. You are encouraged to share them with your primary healthcare provider.

  • These tests are only available at SSCC by appointment only. Monday & Thursday 830am-4pm, Tuesday & Friday 7am-12pm, and Wednesday 7am-4pm.

  • You are responsible for contacting your provider regarding your test results. SSCC providers will not review Self Directed Labs.

What you need to do:

  • Call Sensible Solutions Care Clinic at 715-637-0001 to make your lab appointment.

  • Please have a picture ID available at the time of service.

  • Lab results are most accurate when fasting for 8-12 hours.

  • Come directly to the SSCC location. Check in at the front desk. Inform them you are here for Self Directed Lab Testing.

  • Please note you will be expected to pay for the tests at the time of service with cash or check only.

  • If you have questions, please contact your primary healthcare provider.

  • For a more detailed interpretation of your lab work, request to set up a one time visit with a SSCC provider for only $150 or ask about SSCC membership options.
